Dear Families,

Attached is a copy of all CPS school forms that must be returned to school. We know it is a lot to complete and we appreciate your time. If you download the forms you are able to be typed directly on the form. Please complete all required pages, print, sign, and have your child return to their homeroom teacher. Limited hard copies will be sent home with students.

Page 31 & 32 - Family Income form 

  • This form helps determine federal funds for Garvy. Please Fill out the form completely. All family members in household must be listed in part 1 Form must include the address in part 6 and  the form  must be signed 
  • Families may opt out of sharing their income information but PLEASE Write “opt out” or “not           applying” directly on the form Part 1 and 6 must be filled  and return the form to school and sign the form

School Fee $100 is due by September 2nd. Payment can be made using this link. If you do not know your child’s school ID, you may enter zeroes.

These are on site programs offered by CPS, to be completed at school,  but not required to be returned unless interested.
