Section 2A - Required Documents

Provide the appropriate documents and download, fill out, and scan all health forms below.

 

Driver’s License

Please provide a copy of your license that shows your current address within our neighborhood borders.

 

Two Current Proofs of Address

Please provide documentation that MATCHES YOUR DRIVER'S LICENSE, such as...

  • Utility Bill
  • Recent Mortgage Statement within the last two months
  • Current notarized lease/rental agreement

 

Child’s Birth Certificate

Please provide a copy of your child's original birth certificate.

Section 2A- Required Documents 
 
The following documentation is needed for the registration process:
  • Original Birth Certificate (Child Needs to be 5 years old, on or before September 1, 2026). Be sure there are numbers after the 112 in the right hand corner.
  • Proof of Residency- Two forms needed- ex. Valid Driver's License, Utility Bill, or Mortgage Statement. 
 
Printed copies of the registration packet are available for pick up in the main office.  If you would like to print the packet on your own, please print double sided.
 
GARVY OFFICE HOURS:  8:00 a.m. - 3:30 p.m.
REGISTRATION TIMES:  9:00 a.m. - 11:30 a.m. 
 
Section 2B- Required Medical Documents
 

Download, fill out, and scan all health forms below. Medical Records (dental, vision, and physical) are due the day a newly enrolled student begins school. We will accept medical records sooner. Please click here ​​​​​​​for more details on which medical records are required.
